The best way to ensure an open honest and transparent reporting culture in any business is to install a no blame culture. Anyone reporting an incident must be treated fairly with no disciplinary action unless the act was either gross misconduct or willful. If an incident is not reported, the worker will find themselves facing disciplinary action.  You will need to warn your leaders, board of trustees, there will be a spike in reported incident, it won’t mean you’re suddenly having more incidents, it will mean everything is now getting reported. This will improve the over all culture in with workplace. I have successfully implemented this on numerous tier 1 & 2 projects, over the past 10yrs. In fact, our client (1000 + employees) is now adopting the same philosophy, where 10 mths ago they rubbished it.
